Upon enrolling your child, parents will complete a Student Health Care Summary Form. On the form parents are asked to provide information regarding your child’s medical history. To support our students’ safety, please ensure that all allergies and medical conditions are stated.

Medication must be stored in the administration office except for Epipens which can be kept in the classroom with the child’s teacher and asthma puffers which can be kept with the child.

If your child needs any medication, be it short term or long term, you need to contact the administration office and complete the appropriate paperwork.
